REQUIRED FILING DOCUMENTS

• Proof of Identity. This can be a state issued driver's license, passport, or

other government issued IDs. We need a copy of the front only.

• SSN. This is required and often available on W2s. If your W2 DOES NOT

have a COMPLETE number, please submit a copy of your driver's license

for all NEW clients. DEPENDENTS: all dependents being claimed must

have a birth certificate and social security card on file.

• INCOME STATEMENTS. (W2, 1099s, etc)

• Capital/Cryptocurrency statements

• IRS issued Identification Protection Personal Identification PIN

• Summary/Reconciled incurred expenses not reimbursed by employer (if

applicable).
